Successful Transition to Recovery
Addiction Directions Accountability Program is a unique and innovative program where men and women who have successfully completed rehabilitation program can begin their transition to recovery. This is accomplished by a custom designed, four phase program that includes monitoring, accountability, follow up care, and collateral contact communication. Every person’s situation is different, and at Addiction Directions we recognize that means a specific, individualized plan is required that ensures the best possible outcome. Throughout each phase, sobriety is verified and documented through urinalysis and/or breathalyzer, and a written development note addressing the participant’s overall progress in recovery is provided to collateral contacts by the participant’s recovery advocate on a monthly basis.

How it Works
- Phase I- Transition
In the transition phase, the Recovery Advocate is assigned and develops a relationship both with the participant and the family. A continuing care plan is developed, or if already established, adopted, that identifies the overall history and any specific relapse triggers of the client. Outside support services that will need to be in place for the client are identified and initial meetings are scheduled. The Recovery Advocate assists in developing a 7 day transition home plan, and ensures that all program expectations are clear and all documentation is in place.
- Phase II- Re-Entry (Months 1-3)
In the re-entry phase, the Recovery Advocate continues to work closely with the participant as well as all collateral contacts. Close attention is paid to the reduction of environmental triggers the client may be encountering. The recovery network that the participant is developing is checked and verified. Any family or other social integration issues that may be arising are addressed.
- Phase III- Stabilization (Months 4-6)
In the stabilization phase, the participant continues to be accountable to his Recovery Advocate as well as his other support systems, which by this time, are widening. Relapse prevention is still in place through monitoring and accountability. The participant is also encouraged and assisted in developing a personal development plan, in an endeavor to incorporate financial, emotional, social and recreational aspects of his life into his recovery.
- Phase IV-Maintenance and Growth (Months 7-12)
In the final phase, of maintenance and growth, the participant is focused on enhancing their personal development plan, where areas of personal life that are of paramount importance are addressed, and measurable and attainable goals are put in place, At this stage, any unresolved medical or legal issues are addressed, and health and fitness are brought more to the forefront of the participant’s awareness.
